If you have been referred for more treatments or tests, you can check on the referral using our online form. You will need a letter of referral from your GP if you would like to see a private specialist or consultant.

You can book a referral through the NHS referral service. You can also check a referral by using the NHS website or App.

You will need your NHS account login details and the NHS App to access the App.

First Light

First Light is a charity supporting people in Cornwall, Devon and Wiltshire who have been affected by domestic abuse and sexual violence. In 2017 Twelves Company and Skoodyha merged to become First Light. They are independent of statutory agencies, including the Police, and every year they help thousands of men, women and children in the South West to lead safer lives, free from violence and abuse. Plymouth Healthy Futures Programme (Social Prescribing)

Healthy Futures works with 6 Primary Care Networks in Plymouth covering 33 GP surgeries. They work with patients who would like support to access activities and services in the community to improve their health and wellbeing.  They are funded by Primary Care Network, Plymouth City Council and the Department of Health.  

If you would like some support to improve their wellbeing, a referral can be made to Healthy Futures by your surgery or you can self-refer by contacting them yourself.  If you would like to access their services please follow the link below.

Health & Wellbeing Coaching

We have a Health & Wellbeing Coach that patients are able to access through a clinical referral or self referral.

Health and wellbeing coaching offer 1:1 sessions, led by you, to support with setting goals and taking steps to improve your wellbeing.  

This can be managing a health condition, doing things you enjoy or making positive changes to your lifestyle.

Options Counselling

You can self-refer for counselling through the anxiety & depression service (Plymouth Options) by calling their team on 01752 435419 or emailing plymouthoptions@nhs.net. Alternatively, you can follow the link below and complete their online form.

Specsavers Hearing Tests

You can now self-refer to Specsavers for hearing tests.  To do this you need to call 01752 253434 where an appointment can be made in one of the following locations:
